Employer Declaration & Acknowledgement
By submitting this form, I confirm that I am engaging R.C Employment Pte Ltd to source, process, and submit an application for the employment of a Foreign Domestic Worker (MDW) on my behalf. This submission constitutes a binding agreement between myself and the Agency, including acceptance of the stated service fees and terms.

I declare that all information provided in this form is true, complete, and correct to the best of my knowledge. I understand that any false, misleading, or incomplete information may lead to the rejection or revocation of the MDW’s Work Permit, and may amount to a violation of Singapore laws and/or Ministry of Manpower (MOM) regulations, for which I may be held legally accountable.

I acknowledge that R.C Employment Pte Ltd collects this information based on my representations and instructions. The Agency has limited capacity to independently verify the information provided, and I agree to fully indemnify and hold the Agency harmless from any claims, penalties, investigations, or liabilities resulting from the submission of false or inaccurate data.

I understand that the biodata, employment history, and background information of the MDW is provided by third-party recruiters and/or the MDW. While R.C Employment Pte Ltd exercises reasonable due diligence in reviewing such information, the Agency does not guarantee its accuracy and shall not be held liable for any misrepresentations, errors, or omissions in the biodata.

I further acknowledge that the selection of the MDW is based on my own criteria and preferences. Any subsequent change in household arrangements, job scope, communication breakdowns, or dissatisfaction with the MDW shall not constitute grounds for refund or replacement, unless such entitlement has been expressly agreed in writing.

Even in the absence of a physical signature on this form, the biodata, or the employment history, I agree that by authorising R.C Employment Pte Ltd to submit the application—whether verbally, digitally, or via SingPass—I have acknowledged receipt and understanding of the MDW’s information, background, and past employment history as made available through the Agency’s website, email, or digital communications. I confirm that I had the opportunity to review the relevant details before instructing the Agency to proceed.

Refund Policy
In line with MOM regulations, R.C Employment Pte Ltd is not obliged to provide any refund of service fees or deposits once the application has been submitted and/or an In-Principle Approval (IPA) has been issued, unless otherwise stated in writing. Refunds, if offered, shall be at the sole discretion of the Agency and subject to terms and timelines agreed upon in the service agreement. No refund shall be due in cases of change of mind, misalignment of expectations, or non-disclosure of household conditions by the Employer.

Consent to Data Use
I consent to the collection, use, and disclosure of my personal data by R.C Employment Pte Ltd for the purposes of processing this application, providing after-sales support, and compliance with regulatory requirements. I further consent to being contacted by R.C Group SG and its affiliated entities, including but not limited to R.C Weddings Pte Ltd, Huttons Asia Pte Ltd, and R.C Connexions Pte Ltd, regarding related services that may benefit me.

Binding Consent via Digital or Verbal Submission
I understand that submission of this form—whether physically signed, submitted electronically, or confirmed via messaging platforms or government-authorised digital systems such as SingPass—shall constitute valid and binding consent under the Electronic Transactions Act (Cap. 88) and shall be legally enforceable under Singapore contract law.
